My thoughts on Tommy is that he's a man overwhelmed with grief, regret and vengence to the point of madness. As you can see by the pone in his hand, Dick handles most of his affairs and always manages to get in "family time" for Tommy. However, it's the first time going to visit them for Barbara and she feels just awkward.
